There should be several signatures, 2 qualified persons sign off on the main section, and there can be several signatures required on the bottom, those depend on your company structure but would generally be a supervisor and safety officer.
 
Our eewp has one signature required, should the supervisor or safety guy put the reasons for working hot. our permit writes like the people doing the work must say why they can 't shut down.
 
The minimum elements of an EEWP are listed in 70E [2009] Section 130.1(B)(2).

Oddly enough, sign off by a ?qualified person? is not one of the requirements; however, an authorizing or responsible management, safety officer, or owner, etc. signature is required. See Section 130.1(B)(2)(11).

Note: I?m well aware that Annex J suggests the signatures of Electrically Quallified Persons but the Annex isn?t part of NFPA 70 requirements. I helped write it originally and Section 130.1, as well; responsible management signatory authority was something I insisted on.
