"Effectively grounded"

When I was younger I had this settled in my feeble brain. Now it's sneaking back in to haunt me.
Industrial enviornment. Open wring installed per 398.30(C)
Plant has a ground ring with the service and every column connected thru a 4/0 cadweld jumper.
Only 3 phases are installed on the insulators. Taps are made per 240.21(B)(4) with tap conductors in conduit. From the disconnects at the end of the tap an equipmemt grounding conductor is added to whatever load is supplied. There is no method of grounding via the tap or tap conduit however the disconect means is secured to the column and a bonding screw installed.
Is this a violation ?
If so, was it ever a permissible installation method?

(THis is the prevelant wiring method in a large iindustrial complex with wiring dating from 1940 to present.)
 
Article 250.136 would give the info your looking for. It directs you to 134. Unless you can find an exception in all that, i don't see where its possible.
