1. Can we combine different medium voltages conductors in a manhole without a barrier?
2. Can I use a 15KV cable in a 4160 V. power source.

(e) In manholes, if the conductors of each system are permanently and effectively separated from the conductors of the other systems and securely fastened to racks, insulators, or other approved supports, conductors of different voltage ratings shall be permitted.
 
Well I read 300 + , I didn't chase every noted article, tells what you can't, and clearly doesn't say what I thought ... but I like to think that way 600 volts or less ...

I haven't the time right now to look up the article, but I believe the rule you are talking about has to do with conductors of different voltage systems being in the same conduit, not the same manhole.
