Electrical Panel Requirement to be 12 inches above grade

Is there a reference in 2008 NEC that says "Outdoor Electrical Panels and pull boxes should be 12 inches above finished floor" ? I searched
but could not find. I guess it can be a listing issue for the NEMA 3R panelboard boxes and pull boxes.

Not always a "finished floor" outdoors to start with.

expected conditions can dictate what is best. If there is expected to be standing water, flooding, snow accumulation, etc. that should be taken into consideration.

Nope, IAW NEC, 312.2 just says 1/4" "airspace" behind panelboard
