elevator wire sizing/OCPD

This is the information I was given:

20 HP elevator type motor (80 starts/hour)
Intermittent elevator duty cycle; 30-60 min.
30.0A rms FLA
90.0A rms LRA
Motor code letter "C" for soft starting
(recommended fused disconnect switch: 60A/3P, dual-element fused @ 45A)

Please help; thanks in advance.

Is it 208V or 480V ???????????? I'm assuming it's 3 phase since a 60A 3P breaker was recommended.
