I do not know af any. In fact, some 4"sq covers have ko's in them

You can buy them with 1/2" ko's,,,but I'm not sure what would stop you code wise.

I would have to say yes but flex is acceptible.

I was just looking over 314.22, to me, would allow the MC to connect to the box cover if the "Exception" requirements are met.
