Shouldn't the reduction in 220.61(B)(2) be based on the current without the reduction of 220.61(B)(1)?

Not IMO, I think the example is correct. 220.61(A) gives us the calculation for the unbalanced loads for the 120v loads. Then 220.61(B) gets more specific and states that we can take 70% off the neutral current on specific appliance and any neutral current over 200 amps. It does not say either A or B so you do it for both
