Existing switch and receptacle outlets are in toilet room cmu walls. New ceramic tile, typical 4" bathroom tile, will be added to cmu face.
As contractors, would you use outlet extenders behind mounting tabs? Do these work and hold up over time? Or should i specify to rough-in a new masonry box in existing wall and fish down mc. existing receptacles are not gfci, new ones will be.

yes, the plastic ext rings work fine. Make sure you have room in the box for the gfci or you will need to change the box for a deeper one or use a wiremold ext. ring.

What is "cmu"?

If existing boxes are flush and tile is 1/4 inch or less thick you don't need any extensions.
