Is it required by code to provide grounding electrode conductor for the pole in addition to equipment grounding conductor? If yes, what article section refers to this?

Often speced, but not required.

250.32 Buildings or Structures Supplied by a Feeder(s)
or Branch Circuit(s).

Exception: A grounding electrode shall not be required
where only a single branch circuit, including a multiwire
branch circuit, supplies the building or structure and the
branch circuit includes an equipment grounding conductor
for grounding the normally non?current-carrying metal
parts of equipment.

250.32 is not quite the correct code section, but I can see how it could be applied.
A pole is a structure, every structure requires a disconnect, with an exception for luminaire poles. The rule in 250.32 can't be applied to any structures but dwelling units as 225 requires a disconnect for all feeders to a structure, except for dwelling unit outbuildings.
Ground rods at lighting poles are called auxiliary electrodes, no rule requiring them, we often see them installed because: the specs require, the engineer requires, the last job required, or I think its a good idea.
