If i have a 400 amp MLO panel and need to feed a 150 amp fused disconnect can I tap off the panel with lugs to feed the fused disconnect? The distance is about 225' away to outside. Thank you!!

From what you said I believe you can but make sure of 240.21(B)(5) as stated below.

Is the 400 amp panel inside or outside?

I get the feeling an outside tap isn't going to apply here. The way the question was worded leads me to believe the 400A MLO is inside and you are trying to run the 150A tap 250' to get to the exterior of the building. For something like an AC unit.

I would try and get a 150A breaker on that 400A MLO or put the fused D/C right next to the panel then run the feeder 225'.
