What is NEC's definition of "Field Connection", 230.82 (TVSS paragraph last sentence before 230.90)? My assumption is that a "field connection" is an installation of equipment on live wires only.

I am not sure I understand the question. The words "field connection" do not appear in 230.82 (last sentence before 230.90) in the last three editions of the NEC. What version are you using, and where does the notion of a "field connection" come into play?

The words "field connection" are not in the code...they are in the handbook commentary.
Transient voltage surge suppressors (TVSSs) have been added to the types of equipment permitted to be connected on the line side of the service disconnecting means. Such applications are permitted only in listed equipment, and the conductors connected to the TVSS device must have a disconnecting means and overcurrent protection. Field connection of TVSS devices to the line terminals of the service equipment is not permitted.
Field connection means a connection that is not made by the manufacturer...it is made by the installer in the "field". It has nothing to do with live wires.
