Floor box plate ID

I don't know if this is the right forum to post this, but I am looking to identify this floor plate. It was installed (according to the tenant) 4 years ago in a new commercial retail TI in Los Angeles.

Can you pull it up from the floor? there should be a lable under it.

Found it, it was a Hubbell.

$60.00 each for the cover plate!

As I said before check out Spider they are not only the manufacturer for Hubbel, they also sell direct in many instances.
