Hubbell and Steel City make them for sure, and I would bet others do to.
Hubbell makes a great verity of them in many price ranges.
What are you looking for, wood floor type, pour in place type or poke through type?

LEW Makes a nice floor box with a heavy brass lid with screw on covers to hide the receptacles $26.00+-; Drawback is that the cordcap is sticking up above the box; I have heard that someone has a floorbox (Carlon or Hubble)that the duplex is recessed enough to have the cord cap recessed below floor level
