floor duct outlets

i have a church that has a floor duct system. They have the doghouse outlets screwed into the floor duct now. They are wanting to have them recessed. Has anybody ever seen a device that i could thread into the old floor duct opening and have the outlet recessed. I haven't seen such a product
 
I doubt if there are any. In general, I don't think most floor ducts aren't far enough below the surface of the concrete to allow room for a recessed outlet.

Yeah I've never seen anything either but the duct is 4" deep so was thinking there might be something
