Would the proper lead attachment from my tester for checking insulation resistance be:

One lead to the red and black conductors together of the cable and the other to the ground?

Tester set for 200 Mega Ohms / 500V

Yes, with thermostat disconnected of course.
 
Why are you trying to do an insulation test? You previously said it was not getting warm. Is it tripping the breaker or GFCI now? If not I see no reason to test the insulation. Something it "open".

Owner is insistent on involving Nu heat technical support.
They are asking for a insulation test.

They want to be sure they can't push the blame to someone else first.

Kind of like when you make a medical insurance claim and one of the first things they ask is if it was from an accident.

Did you ever bypass the thermostat and confirm you are getting 240 volts with the mat connected to the circuit? You said the mat resistance was correct so that points to a supply issue.

Harkening back to my days repairing transmissions:

When a customer comes in with an' extended warranty' on a transmission and the first question is 'Is it leaking?'; if so, no warranty.
