Just my initial thought. I can't believe a recep is allowed under a swinging chair in a dental office!

The problem is not with the swinging chair, The problem is with the person siting on the rolling chair working on your teeth. If the receptical is locateded in the proper place at the front of the chair which in under the foot rest, nobody would be kicking the recpticals.

It would not look very good but you could remove the duplex rec, install a blank cover plate w/ 1/2" ko, emt over to a safe location and install wp box and outlet there.

Then they also have a violation.

314.27(C) Floor Boxes. Boxes listed specifically for this application shall be used for receptacles located in the floor.

There is an exception for show windows if the AHJ agrees, but of course that does not apply here.

If you want to fix this right the boxes must be changed, and as Cavie points out they may just be in the wrong spots.

At the very least a bell box extension (yes they make them) installed on the existing bell box might stop the plugs and covers from getting broken.

I might try finding something like this....

...and figure out a way to install it over the existing bell boxes.

They're replacing the flooring? Fantastic! Cut the floor and move the receptacle, replacing it with the right one.
