Does anybody know of a floor box with a blank cover suitable for outdoor use installed in a concrete deck?
 
Yes, but I don't know the manufacture. They were pretty much the same as for a floor box, but they had a gasket and the cover screws on. I inspected some in a wood deck at a resturant here in town.
 
Are you asking about a junction box or a floor box that devices can be mounted in?

OZ-Gedney makes cast steel junction boxed that are designed to be embedded flush in concrete.

What size are you looking for?
