Does a two pole gfci breaker measure the difference in current across all three conductors (a&b phases or legs, & neutral/grounded conductor), or does it pair the neutral with one of the ungrounded conductors to sense a difference? I have always assumed it was the first method because i cannot comprehend how the second would even work (given that a portion of the load would be phase to phase/leg to leg and the neutral would carry only the unbalance, it seems to me that any time there was a line to neutral load as well as a line to line load there would be a difference.)

Yes and if it is a 3 pole GFCI it measures A, B, C, & N.
