Gaps in the Junction Box

I have a guy who used a standard octagon blank cover on a concrete junction box. The result has gaps. He should have used a concrete box cover plate. Can anyone remember where the Code spells out the maximum gap allowed? or another Code that would have him install the correct cover?

See 314.21 Repairing Noncombustible Surfaces.
Noncombustible surfaces that are broken or incomplete around boxes employing a flush-type cover or faceplate shall be repaired so there will be no gaps or open spaces greater than 3 mm ( in.) at the edge of the box.
 
314.25 requires a cover plate but does not specify any spaces left around the cover.

110.3(B) would apply if a 4/0 cover is most likely only listed to be used on a 4/0 box.

Here's what we came up with in the office:
314.28 (C) ?covers compatible with the box?, which this is not. 314.41 also states covers ?listed for the purpose? which this is not.

I agree, that one nails it.

A 4" octagon cover is not compatible with a deck box.
