Are you required to have a ground rod installed on a garage sub panel
The garage is detached from house
House has 200 amp panel and wire running to garage is is 4 wire leaving house underground to garage 100 amp panel separating grounds and neutrals

Yes a GES is required at a separate structure when you have a panel there. You would need two rods for the GES or a single rod tested to be 25 ohms or less.

A grounding electrode system is required. That can be any acceptable electrode, a water pipe is supplemented with another electrode, but is most commonly a ground rod, or two
