GFCI requirements for residential kitchen and hydrotub

Are all outlets located on the walls above a residential kitchen countertop required to have GFCI protection (regardless of the distance from the sink)?
Is the requirement for GFCI protection for a Hydrotub a new requirement (my tub was installed in 1993, and I'm not sure if it has one)?

Additional questions regarding GFCI -

If all the outlets above the residential countertop are required to be GFCI protected, was this a requirement in 1993?

Was there a GFCI protection requirement for hydratubs in 1993?

If these questions have been answered previously, I apologize for not being able to find them - I did search for this information but was unsuccessful (this is my first time to use this website).

I appreciate any help or answers I can get.

GFCI.

1981 NEC tub motors.
1987 NEC required receptacles serving kitchen sinks.
1996 NEC receptacles serving all kitchen counters.

1993 NEC:
210-8(5) required GFCI within 6' of kitchen or wetbar sink
680-70 required GFCI protection for hydromassage bathtubs
