On all 120V recptacles rated 15 and 20 Amps. See210.8(B)(3) 2002

Note this has no exceptions like that of a dwelling.

210.8 Ground-Fault Circuit-Interrupter Protection for Personnel.

(B) Other Than Dwelling Units.
All 125-volt, single-phase, 15- and 20-ampere receptacles installed in the locations specified in (1), (2), and (3) shall have ground-fault circuit-interrupter protection for personnel:

(3) Kitchens
As Joe pointed out, there are absolutely no exceptions in commercial kitchens.

A 20 amp 120 volt outlet even if located behind a 1000 lb refrigerator is required to be GFCI protected, I might use a GFCI breaker on that one. ;)
