Looking to see opinions on the differences
between the requirement of 230.95 and the exception of 240.13.(2).
The 1st says you "shall" provide at the service disconnect, while the 2nd sounds like you don't have to, if protection is provided downstream.
 
The rule in 240 covers applications, such as second buildings where an upstream GFP device could provide the required protection for the second building disconnect. It does not modify the requirement in 230.95, that the service disconnect have GFP protection. The intent is to protect the service disconnect from an arcing ground fault within the service equipment and a downstream GFP device could not accomplish that.
