Attached sketch shows incoming electric utility service feeding 1200A, 480/277V service disconnect fused at 1200A. The service disconnect feeds panel MDP1, 1200A 480/277V. MDP1 has (3) 200A feeder breakers and (1)600 A feeder breaker.

Their is no GFPE at the 1200A service disconnect. However MDP1 has GFPE in its feeder breakers.

The question is GFPE required at the service disconnect and at the main breaker of MDP1?

I see since MDP1 feeder breakers are not more than 1000 amps GFPE is not needed.

Based on 230.95 GFPE is required for service disconnect.

What if MDP1 had 1000 amps feeder breaker then installing GFPE in MDP1 would still not be code complaint I am assuming?

Pretty clear why, you only have one and it's not protected.

230.95 Ground-Fault Protection of Equipment. Ground-
fault protection of equipment shall be provided for solidly
grounded wye electric services of more than 150 volts to
ground but not exceeding 1000 volts phase-to-phase for
each service disconnect rated 1000 amperes or more.
