Ground Conductor

Would you need to add a ground as they indicate?
It's from the utility manhole to the service switch. HDPE is a non metallic pipe so I'm assuming a ground is needed. 3 phase 120/208v service

No there is no EGC required (or permitted) for service entrance conductors. You only need the 4 conductors mentioned in the note.
