Ground Fault Protection of Breakers 1000A or more

Are all breakers rated at 1000A or more required to have Ground Fault Protection, or only service/building mains?
1. If I have a 3000A main with GFI protection at my service, and then have 1200A and 1000A sub-mains, do they also need GFI protection
2. If I have a 1000A main wtih GFI protection feeding a 750kva transformer which feeds a 2400A main, does the 2400A need GFI protection?
 
Services and feeders over 150 volts to ground not exceeding 600 volts phase to phase. There is an exception for breakers which have GF protection ahead of them, but selective coordination might mandate GF on the downstream breakers.

For the record it GFPE not GFCI. Take a look at (2):

240.13 Ground-Fault Protection of Equipment. Ground-
fault protection of equipment shall be provided in accor-
dance with the provisions of 230.95 for solidly grounded
wye electrical systems of more than 150 volts to ground but
not exceeding 600 volts phase-to-phase for each individual
device used as a building or structure main disconnecting
means rated 1000 amperes or more.
The provisions of this section shall not apply to the
disconnecting means for the following:
(1) Continuous industrial processes where a nonorderly shut-
down will introduce additional or increased hazards
(2) Installations where ground-fault protection is provided
by other requirements for services or feeders
(3) Fire pumps
