Ground Fault Protection has a complete listing in the Index of the Code Book.

I guess it depends on what the service is and and what the service will

not sure what you are asking but here are two comments

ungrounded 480 volt service needs ground fault detection

480 volt main breakers 1000 amp and over need GFCI protection
