I run into this problem fairly often.
where can the contractors download a ground rod test report form, I am getting tired of doing their leg work for them.

Are you an oil rig inspector?

Is this reading you want from the rig or just the service (because the rig is usually bonded to the service)

If so, I would make a certificate showing all info and have them sign or seal/sign it.

Strange, I would have thought the state would have their own forms for whatever they require.

Maybe it's something like the bonding certificate needed in NJ for public swimming pools ? In NJ it just needs to say it was done by a licensed EC or a licensed accepted testing agency and seal pressed with the name, location etc etc etc Ive seen some really official looking ones and Ive seen ones written out by hand. I should check the bluebook on that.
