Ground Wire For Service?

Does the code require a ground wire be included with the underground service entrance conductors (from utility company transformer to the building service equipment)? Could this constitute a ground from a seperately derived system?

In other words, the neutral, which must be run to the service disconnect, is the ground. Up to and including the main (first) disconnect, the ground and neutral are the same conductor.
