Ground Wire in a Concrete-encased Duct Bank

This is a quick question, but I can't seem to find it anywhere. Would a grounding wire be required by NEC code in a concrete-encased duct bank?

Thanks to whomever can help.
 
If it's carrying a feeder circuit, then an equip ground conductor is required in the raceway. If it's a service lateral from the utility transformer, then a ground wire is not required.

For reference, what is the section of the NEC where this can be located?

250.120 Equipment Grounding Conductor Installation.
An equipment grounding conductor shall be installed in accordance with 250.120(A), (B), and (C).

If it's a grounding electrode conductor...

250.64 Grounding Electrode Conductor Installation.
Grounding electrode conductors at the service, at each building or structure where supplied by a feeder(s) or branch circuit(s), or at a separately derived system shall be installed as specified in 250.64(A) through (F).
