ground wire in duct bank

I have a site that lef out the ground conductor wire in the ductbank. How do I fix this problem? Can I install the ground wire outside the envelop?

I hope this duct-bank is not enclosing service conductors since most electric utilities will not permit grounding conductors in their transformers. We do not even permit metallic conduit to enter a transformer. :-?
 
Are you referring to the grounding conductor for the overall duct bank itself or the grounding conductor that was suppose to be within the individual ducts (individual phase conductor wiring) within the duct bank.
