Ground Wire in Metallic Conduit

Where do I find information about a service ground wire in conduit, and how you might need to use a bonding bushing on both ends?

I don't ever remember using this application...

A note on Service Ground Wire. The NEC has very specific terms for "grounding". We as electricians, often call them all ground, which is why many are confused about grounding and bonding. Definitions ar listed first in Article 100. Knowing the definition points us to the rules.
