grounded receptacle installation

Don't know if this subject has been discussed ( new to site ) , but , I have been curious to know if anyone else has concerns regarding the installation of grounded receptacles , i.e. , "ground up or ground down ".

It is my belief " ground up " is more appropriate because a partially inserted plug exposes hot to neutral ( or hot to hot ) blades to falling objects.

Any comments would be appreciated.
