Grounding electrode conductors

Two questions here...

1. If you run your GEC in conduit, do you need a bond bushing or ground hub on each end of the conduit?

2. Does your GEC ever have to be bare copper?

1. Metal conduit- Yes. PVC conduit- no

2. No. But sometimes it has to be copper.
