Grounding subpanel

I have a ground wire in the meter base and outside disconnect that are attached to the neutral feed and an 8 foot ground rod. The 4 wires enter the breaker panel, the 2 hot leads go to the main breaker the neutral and ground are bonded at that point. Should they be separated at this point? There are also 3 wires that go to a breaker panel in an unattached garage, 2 wires to the main breaker and a 3rd wire to neutral and the ground. Does a 4th wire for ground need to be run or can a ground rod be driven at that point and the neutral and ground separated.

Should a steel building be bonded to the breaker panel or should ground rods be driven and the build attached to them.
