grounding transformer

I ran across this today. 480 volt 3 phase transformer fed from MDP. transformer feeds 208 volt 3 phase panel.
transformer rated 1600 amps.
no ground conductors. transformer not grounded.
XO not grounded to frame. no grounding bushings on conduit.
I use to ground XO to frame. grounding bushings grounded to frame.
what is the proper way to ground transformer?

250.30 is where you need to look. You can bond the XO in the transformer or at the first disconnect.
