Grouping of disconnects

Upgrading a 200A residential service to 320/400. 2017 code cycle. The scenario:

- Meter base fed from mast.
- To left of base, 200A disco for pole building and 4 wire feeder out.
- From bottom of meter base, 4/0 AL to existing 200A MB panel right inside basement.

Two questions.
- Does this violate 230.72 grouping of disconnects, as one is outside and the other inside? Inspector already pre-approved the drawing.
- Should I run a #6 from the MB panel, then to disco, and then to rods, all continuous? He will only have two ground rods as electrodes, his well pump and literally all his plumbing is plastic/pex.

Locally that would not be allowed per 230.72.
You might note 230.40 exception #3 would allow your supply to the detached structure to be fed from the meter without the disconnect at the meter.

Yes, but we have an overhead tap down the line to a shed near the pole barn, so we had to go 4 wire.
