Does anyone know if there is any difference in light output when comparing 4" can lights; one being a 50 watt 120V halogen lamp (with medium base) and the other being a 50 watt 12V MR-16 lamp?

Certainly the distribution will be very different particularly depending on the spread selected. Visit the lamp manufactures websites for details and answers to your question. I like Osram Syvania's site, but GE and Phillips have websites as well.

It looks like they are somewhat similar if you compare a PAR30 to a MR16.

For the 50W PAR 30, you get 13000 CBCP with a 9 degree beam angle.

For the 50W MR16 you get 10,500 with a 11 degree beam angle.

So the PAR 30 is a little brighter at its center point, but the MR16 has a slightly wider angle.

This is from a Sylvania catalog. And of course, you can get either with wider beam angles.

I agree. I've been comparing them as well and for example if you compare the 25-degree spread both lamps have 3000 CBCP. I would still run the lighting calculations for both though because I think the lighting levels would vary since one lamp is much larger than the other and the way the light escapes the luminaire would also vary.
