I cannot compare what Mike Holt said because I do not know what material you are referencing.
It is my understanding that you only use handle ties when you are supplying line to neutral loads.

I think you must be misquoting the handbook.
Section 240.20(B) covers these requirements.
(1) states that except where limited by 210.4(B), individual circuit breakers, with or without approved handle ties, shall be permitted.....that serve only single-phase line-to-neutral loads.
(2) states individual single-pole circuit breakers with approved handle ties shall be permitted for each ungrounded conductor for the line-to-line connected loads.
(3) states that 4-wire, 3-phase systems can use approved handle ties with individual single-pole breakers for line-to-line loads.

I dont have the handbook, but I am guessing you are refering to section 230.71(B).
This section allows indivdual single-pole breakers on multiwire circuits only to be handle tied in order for it to be considered one disconnect as opposed to two separate operations.

handle ties are not allowed where it is required for over current protection to disconnect more than one of the ungrounded conductors simultaneously.
430.38 Number of Conductors Opened by Overload Device.
Motor overload devices, other than fuses or thermal protectors, shall simultaneously open a sufficient number of ungrounded conductors to interrupt current flow to the motor.
