hanging fixture height

Does anyone have any suggestions on what height to hang a chandelier in a foyer with 19' ceilings. Most times there is a window that I center the fixture in so it can be seen from the outside, not so here.
The input I will get from the customer will be "do whatever is standard". Is there a standard height for this scenario?

It will also depend on the size, style of the fixture and the size of the room. Just hang it and see how it looks.

I usually have it about 12" above the door jam.
 
What Edward said. I will add "What looks good to you". Is changing the lamps a consideration as far as height ? Is there a balcony on the second floor to access the fixture ?
 
In my experience, exact height depends a lot on the particular fixture you're hanging, but one general rule of thumb I try to observe is to hang them at a height which can be reached from an 8' stepladder for relamping by the homeowner.
