Hardwired cooktops and double ovens, require a lockable disconnect if not in sight of appliance (110.25). Does 422.34 unit switch(es) apply to these type of appliances if the ungrounded conductors remain connected to the appliance or should a lockout device on the circuit breaker be installed?

If a unit switch does not disconnect all ungrounded conductors it does not qualify for 422.34. Without a separate disconnect at the unit, in order to use the circuit breaker as the disconnect, you would have to provide a lock off device at the breaker that is not readily removable or transferable.
