Help - Code Reference for Line Side Terminals on Top

Too early for full brain function. I believe there is a Code rule for the upper terminals in a disconnect to be the Line Side Terminals. Does anyone remember which Article this is?

I think I answered my own question by Googling old Mike Holt threads. Only a requirement if the CB is marked Line and Load OR knife switches depending on orientation. Did I get this right?

FYI, and jim can correct me if this is wrong, but I think your references would be 404.6 for the switch and 110.3(b) for the breaker.

Too lazy to look, but of it is a fused disco, isn't there a requirement that the fuses not be energized when in the off position?
