It's a Sumitomo Japanese motor.Can anybody help me identify this motor wiring-box cover and locate a replacement?
Google Sumitomo Electric Group or Sumitomo Junkyard .com.
One of the biggest electrical /mechanical mfg in Asia along the lines of Mitsubishi and GE.

Use a general purpose or even NEMA 4 or 12 enclosure and fabricate to fit the motor?
Done that a few times, especially for over ~10 hp motors. Priced new motor box for 100 HP one time, and decided to just make our own - no waiting for it to ship was part of the decision making process.
