When installing this system we torqued all connections to spec and now 4 months after installation the A - Phase connection at the breaker/bus bar adaptor has "welded" itself. I've never seen something like this; there is no smoke or soot. Is it possible for a surge to cause a "melt down" like this? I normally would suspect a loose connection, but this was torqued once and checked twice. Additionally, it seems odd that there is no burn marks or soot. This is a 3 - Phase NQ Panel Board with a 200 Amp Main Frame breaker.[/ATTACH]
The panel has a light load on it currently, so my plan is to re-feed the line side to the panel bus after I remove the Main Frame breaker and send it off to be evaluated.
Please let me know your thoughts - I'm perplexed and extremely concerned over this.
