Help with grounding and bonding with main panel.

Hello, I am new to this type of posting so I hope I'm going about this correctly. I am getting ready to rewire a main service panel in a small house I have purchased. I work as a industrial maintenance tech. and have done quite a bit of electrical work over the years but haven't been able to find a definite answer to my question, even from licensed electricians. The panel is inside the house with a main breaker. It is fed from a disconnect beside the meter can on a pole from around a 100 feet away. Does this mean that the main panel in the house should have the neutral and ground isolated and the pole neutral an ground bonded at the disconnect. I have installed subpanels and know that they should be isolated after the main panel but haven't face this before.
