Help with Identification

Hi. This isn't really a code question per say, but i need help identifying what these are and if possible where to get them. This it the only site I really go to to communicate with experts.
First up is a metal guard/lock out device for a switch.

I made the cover for the box. But the metal device around the switch is what I am concerned about.
I found another on what looks like a pre-made box, a square D brand, so I don't know if that is where the other came from or if can be purchased separately.

The other is a watertight PVC switch cover. It has a pull type mechanism that is PVC. I found one in my plant, but has no numbers or brand on it.

We have these Carlon switch covers. The problem is the red part breaks, to the point where it's unusable after.

The handle guard you show is from Square D, it is an accessory for thier Class 2510 Type F manual starter.

I once bought the metallic cover from Appleton or Killark. This is a standard design from the manufacturers of cast "explosion proof" enclosures

I made that cover.
I had been wanting to use up that box, and that was the perfect place.
