Help with Lutron DVSCSTV dimmer in 4 way

Long time lurker, first time poster. I've been helped by this forum so many times, I figured it was time to register. I am stumped by a wiring issue in a new addition a built. Installing a Lutron DVSCSTV dimmer as the first switch in a configuration. Both incoming power and lights connect to this switch. from this switch, I have 14/3 to a Lutron SC4PS-SW 4-way switch and then 14/3 from there to a Lutron SC3PS-SW. I'm following the below diagram. When done, the lights work at each location on/off, but if either of the 3/way or 4 way are off, the first switch will not turn the light back on. What am I missing?

I realize you are an engineer but we cannot help with DIY. I will say this, not all 4 ways will wire as your diagram shows. Usually the box has a diagram on it.
