Customer wants a 6 “ can fixture removed and in replace install a pendant fixture (light weight) I was gonna do a round cut in box but obviously won’t work with the 6” hole in the ceiling. A spreader bracket between joists in attic maybe? Then use a medallion to cover the oversized hole that the canopy from the pendant won’t cover.? Any other ideas
 
I was going to suggest the medallion under I read down further in your post. I've have done the same thing in the past. If the medallion is large enough you can cut out the ceiling to get to the ceiling joists for mounting the box. Or I would just use a fan box designed for existing ceilings and cover the hole with the medallion. The NEC would require you to patch around the box even if it's covered by the medallion.

Well that’ll be up to the home owner but as far as the nec goes I’ll use a foam fire block spray instead of patching
