Very early on I think there were some 30A services, a 60A fuse box was very common for quite a while. There were some low amp single leg services put in early on too.

Is this a historical or modern installation question?

Luc6453,the smallest i remeber is a 15 amp glass plug fuse in a basement 6 stories below the apartment.....God I feel old now LOL.

Good question. I couldn't find anything specific in the code but 100A will cover it, no less.
