The simple answer would be "like any other conductor" ie: determine what equipment is connected to the high leg, however, if you are dealing with a main breaker service, the size of condcutor on all phases will be determined by the load AND the service breaker size.

With the 100a minimum service size, and future additions, I'd say at least #4 CU.
